the best bet is write out your own
then have lawyer look it over, figure $150.00
I got my ideas from automotive invoice from OEM dealer. but in the end you decide what to put.
Also consult with a collection agency what need be on the paperwork in the event you need their help-- this will be free and good place to start
as for warranty
Simple IDEAS but not suggested wording or content -
Warranty only covers defects parts and all warranty as per the product manufacture and subject to payment in full and reimbursement upon manufacturer OK and payment of replaced defective part.
The repair is not a rebuild and additional needed work will be on a Time and material basis. All work without service contract is time and materials a service contract is suggested.

i use a invoice that several companies sell one of which is nebs. its the "product 313" "technical service order". it has all the warranty stuff already on the back, the front is perfect for our line of work. just google that company and you can look at it online. Jim
